Images Used:

I created the images in Photoshop. You can create the entire “widget” form and then cut it up using slices. Save your slices using File->Save for Web and Devices. Next select the slices you want to export as images by shift clicking them with the slice selection tool and hitting Save. In the next file dialog choose:
Format: Images Only
Slices: Selected Slices
